Univera Healthcare announces 2026 Health Equity Innovation Award recipients

BUFFALO, NY – Univera Healthcare has announced the recipients of its 2026 Health Equity Innovation Awards, an annual funding initiative that supports nonprofit organizations working to eliminate health disparities and improve health outcomes across Western New York. The awards recognize organizations that are driving meaningful progress through innovative, community‑centered solutions designed to meet the unique needs of the people they serve.

Five nonprofit organizations were selected from more than 60 applicants. Their proposals were chosen for their innovation, community engagement, and alignment with the goal of advancing health equity. The selected organizations are implementing solutions tailored to the needs of their communities, with a focus on measurable impact and long‑term sustainability.

Community‑based organizations across Western New York report increasing strain in meeting basic needs, as rising living costs, limited access to health care, and longstanding disparities continue to shape residents’ health and well‑being throughout every stage of life.

To ensure the awards addressed the most pressing health challenges, Univera Healthcare applied a data‑informed, region‑specific strategy. Drawing on community health assessments and stakeholder input, the organization identified areas where disparities are most significant, directing funding to initiatives with the greatest potential to improve outcomes.

Award recipients will use the funding to implement a range of community‑based programs focused on improving health outcomes and advancing equity across Western New York. This initiative reflects Univera Healthcare’s commitment to strengthening community health by investing in programs that promote equity, expand access to care, and support underserved populations.

The awards described below are primarily funded by Univera Healthcare, with some awards also receiving supplemental funding from the Health Foundation for Western & Central New York. This added investment enhances the overall impact of the awards and supports expanded community benefit.

Health Equity Innovation Award Recipients

American Heart Association – Creating Community and Clinical Linkages for Blood Pressure Management

Helps people check and manage high blood pressure in community settings. The program offers screenings, education, and connects people to follow‑up care at The Chautauqua Center.

Resource Council of Western New York – Food Security Programs

Provides year‑round access to healthy food for Buffalo’s East Side residents. Families, seniors, and children can choose foods they prefer from pantries, farmers markets, and community distributions.

Niagra Falls Memorial Medical Center-Navigate and Nourish Program

Helps mental health patients maintain food access during SNAP benefit changes. The program offers one‑on‑one assistance with benefits and basic nutrition guidance.

ECMC Foundation – Improving Post‑Discharge Outcomes for High‑Risk Patients Through Enhanced Care Transitions and Access to Communication

Supports patients for 30 days after leaving the hospital. Patients receive help with follow‑up care, transportation, medications, and phone access to stay connected to providers.

Community Wellness Works – Care Access Mobile Lab Initiative

Brings lab testing to pregnant and postpartum individuals at home or in the community, helping people overcome barriers such as transportation, childcare, or cost.
